This public website and its intake form are not secure channels for confidential financial-institution information. Do not submit member information, account information, credentials, examination materials, board materials, confidential personnel information, or other nonpublic information.

Information collected

CUelevate may collect information that you voluntarily provide through the external Tally intake form, including your name, work email address, organization, role, optional phone number, and high-level information about the professional support you are exploring.

The website host may also process limited technical information normally generated when a website is accessed, such as IP address, browser type, device information, request time, and security or diagnostic logs.
